Renew a Professional Entity

Professional Limited Liability Company [PLLC])
The North Carolina Psychology Board is pleased to offer online renewals for Professional Entities for the second year.
PLEASE NOTE: WHEN RENEWING ONLINE, you must use a debit/credit card (Master Card, Visa OR Discover Card) WHICH REQUIRES AN ADDITIONAL $5 CONVENIENCE FEE. To renew online, please enter the Professional Entity (Professional Corporation or PLLC) four-digit certificate number below
EXACTLY as it is found on the 2018 Renewal Application for Professional Corporation or PLLC that you received from the Board via USPS. If the system does not initially recognize your certificate number, please check your 2018 Renewal Application for Professional Corporation or PLLC, and re-enter your four digit certificate number below, including any leading zeros, but no letters.
If you do NOT use a credit card, proceed to the RENEW BY MAIL link.
CHANGES OF INFORMATION: If you renew online and any information on the pre-populated 2018 Renewal Application for Professional Corporation or PLLC form has changed, please make the changes on the form, and send the form to the Board office in ONE of the following ways:
- MAIL the Renewal Application with noted changes to the Board office at 895 State Farm Road, Suite 101, Boone, NC, 28607;
- FAX the completed Renewal Application with noted changes to (828) 265-8611 (no cover sheet is required);
- SCAN and email the Renewal Application with noted changes to firstname.lastname@example.org.
